 TO DOWNLOAD A PDF  COPY OF THE ENROLLMENT FORM: (Using Internet Explorer 7+)
  • Click the Adobe PDF icon on the left.
  • A new window will open.
  • Click "Download PDF Version Now" to open a PDF copy of the form. You can now print it or save it to your computer. Or . . .
  • Right click "Download PDF Version Now" and choose "Save Target As" to download the PDF file directly to a location your choose on your computer.
  If you have difficulties downloading or printing the PDF version of the enrollment form, please click here to send an email to the web administrator or call The Crate Escape office at (630) 579-1220.
 
  You can bring or fax your completed form to The Crate Escape.  Fax to: 630-579-1226.

ABOUT ADOBE ACROBAT:

You'll need Adobe Acrobat installed on your computer to view and print the Enrollment Form. If you don't already have Acrobat you can download and install it for free. Click the Adobe logo below to go to the Adobe Acrobat download page. Once there:

  • Choose your language, platform and connection speed

  • Then click "Download the Full Version" and select the other options according to your preferences (We recommend you do not use the Adobe Download Manager unless you have a specific need to do so.)

  • Click "Continue"

  • On the next page click "Download"

  • On the "Save" window that will pop up, choose a location on your hard drive and click "Save"

  • Once the file downloads, double click the downloaded file and follow the installation instructions

  • You may then revisit this page to open or download the PDF file above

  • From now on, the Adobe reader will automatically open any file that ends with ".pdf" that you download or try to open from the Internet

FYI: The current version of Adobe Acrobat is version 7.0. If you have an earlier version, you may wish to update the application by clicking on the Adobe logo at the bottom of the page.
